Six County Association of Governments (SCAOG) is accepting applications for a full-time certified case manager (CM) to join our Aging Services team for in-home service programs. The CM will provide case management for the Caregiver, Alternatives, and the Medicaid Waiver Programs.

The CM will conduct risk assessments for in-home programs, arrange for services of contracted providers, and review client billing for accuracy. The CM will be responsible to document all activities and keep accurate and complete client files and records. The CM will meet with clients in person and electronically throughout the SCAOG region.

Applicants must be self-motivated and able to work with little or no supervision. Must have good organizational skills and the ability to communicate and work well with the aging population. Attention to detail, organizational skills, and computer experience is required.

Applicant must have a valid driver’s license and be willing to travel throughout the SCAOG region which includes Juab, Millard, Piute, Sanpete, Sevier, and Wayne counties.

Applicant must have a four-year degree. RN, CCM, or SSW certification is required, or must be completed within six months of hire. This is a full-time position with full benefits.

Starting salary is $19.00 to $21.00 per hour depending on certifications and experience.
