R6 Regional Council (formerly known as Six County Association of Governments) is accepting applications for a full-time certified case manager (CM) to join our Aging Services team for in-home service programs. The CM will provide case management for the Caregiver, Alternatives, and Medicaid Waiver Programs.
The CM will conduct risk assessments for in-home programs, arrange for services of contracted providers, and review client billing for accuracy. The CM will be responsible for documenting all activities and keep accurate and complete client files and records. The CM will meet with clients as required both in person and remotely.
Applicants must be self-motivated and able to work with little or no supervision. Must have good organizational skills and the ability to communicate and work well with the aging population. Attention to detail, organizational skills, and computer experience is required.
Applicant must have a valid driver’s license and be willing to travel throughout the R6 region which includes Juab, Millard, Piute, Sanpete, Sevier, and Wayne counties.
Certification as a CCM, SSW, or RN is required, or currently in the process of attaining within a couple of months. This is a full-time position with full benefits (Medical, Dental, Pension, 401k, vacation, sick, holiday pay, and more). This is a 40-hour workweek position, but flextime policies allow for a 4-day work week if desired and no weekend work. Starting salary is $22.00 -$24.00 per hour depending on certifications and experience.
